Terms and Conditions

Annual membership dues are $25.00 and due no later than December 31 of each year.. Your membership dues are required when you return this completed renewal form. Mail to: CPAAG Renewal, Attn: Treasurer  PO Box 2244  Glendale, AZ  85311-2244 or give to a CPAAG Board member.

I further acknowledge that my membership in CPAAG may be terminated if I am subsequently arrested for a felony or misdemeanor.  I agree to follow all rules and Bylaws of CPAAG.
